You can select benefits from the Your Wellness Partners Basic Plan1 and add one or more of the following benefits and services to create your own customized offering :
Health Club Plan
The Health Club Plan saves Members up to 60% on monthly dues at over 2,000 clubs nationwide, with month to month membership options available. Members also have access to online Diet Programs, Health Coaching for one-on-one Weight Loss, Smoking Cessation, and much more.
Physician Services Plan
The Physician Services Plan provides referrals to qualified, credentialed physicians through a network of over 350,000 general practitioners and specialists, with fee reductions of 5% - 40%.
Physician and Hospital Referral Plan
The Physician and Hospital Referral Plan provides referrals to thousands of qualified, credentialed medical facilities and physicians nationwide, with reductions of 5% - 40% off on all services. The Hospital Referral Plan benefit is not available in Maryland, as discounts on Hospital services are not allowed under Maryland law. Physician services are eligible for the discounts on the program in Maryland.
Toll-Free 24/7 Nurse Helpline
The Toll-Free 24/7 Nurse Helpline provides Members and their families the peace of mind of knowing that a trained, licensed health professional is only a phone call away to help callers make informed decisions about their health and the medical care they receive. Nurses do not diagnose or provide treatment, but they do provide information about self care techniques for common symptoms, explanations of what to expect during a medical test, and answers to questions regarding diagnostic and surgical procedures, prescriptions and over-the-counter medication information.
Complementary/Alternative Medicine Plan
The Complementary/Alternative Medicine Plan provides referrals to over 30,000 qualified, credentialed alternative medicine practitioners. Discounts of 10% - 30% off the participating provider's standard fees apply on services including Acupuncture, Massage Therapy, Nutritional Counseling, and Meditation.
Podiatry Plan
The Podiatry Plan provides a 50% discount on the initial exam and 20% discounts on all other services and products. The network is comprised of over 3,000 podiatrists.
Medical Records Plan
The Medical Records Plan provides a "safe deposit box" for vital health information and documents, including medical conditions, allergies, medications, emergency contacts, physician contacts, insurance information and advanced directives such as living will and organ donation authorizations. Information can be updated at any time. In the event of a medical emergency where a Member can't communicate, doctors can use a special emergency password to access critical information the Member would want them to see.
Senior Care Search Service & Discount Network
The Senior Care Search Service & Discount Network provides access to over 3,000 highly qualified medical providers of in-home and facility-based long-term care services. Network providers offer savings of 5% - 30% off the participating provider's usual private-pay rates.
Patient Advocacy Program
The Patient Advocacy Program is designed to provide Members with personalized attention in selecting a participating health care provider. Our Provider Relations Specialists will call the participating providers to let the provider, or the office manager, know the Member's name and that they will be calling for an appointment. This process ensures that the provider and/or the office staff know which program the Member belongs to so the Member will be charged the correct fees. This program is not available in California.
Emergency Travel Plan
The Emergency Travel Plan provides 24-hour access to multi-lingual representatives to assist Members with a wide variety of travel related situations such as emergency air evacuation and assistance in obtaining medical care both within the United States and abroad. The program also includes services such as emergency legal assistance, emergency message service, insurance coordination, lost baggage, passport assistance, and travel agency services. This benefit is not available in Florida, Connecticut or New York.
Travel / Hotel Discount Plan
The Travel / Hotel Discount Plan provides a 50% discount off regular non-discounted room rates at thousands of participating hotels and motels across the United States. In addition, Members receive discounts on car rentals from several participating national rental agencies, including Alamo, Avis, Budget, Hertz, and National Car Rental.
Legal Counseling and Referral Plan
The Legal Counseling and Referral Plan provides referrals to a staff of expert, licensed attorneys exclusively dedicated to telephonic consultations. The benefit includes free consultations by local attorneys and a 25% reduction in fees for representation.
Financial Counseling and Referral Plan
The Financial Counseling and Referral Plan provides referrals to a staff of experts, including Certified Public Accountants (CPAs), Certified Financial Planners (CFPs), and other professionals exclusively dedicated to providing telephonic financial information.
GuidanceResources® Online Plan
The GuidanceResources® Online Plan provides access to expert information and tools to assist Members with issues that matter most to them. Members can research in-depth information regarding thousands of topics, from home repairs to financial planning; search to locate qualified providers; download audio and video on featured topics; and have access to online tools, such as financial calculators and online self assessments.
Identity Theft Plan
The Identity Theft Plan provides Members with access to prevention, detection, and recovery services such as triple bureau credit monitoring with automatic activity alerts, unlimited triple bureau credit reports, and instant access to their triple bureau credit score. In addition, Members receive up to $25,000 of identity theft expense reimbursement insurance (not available in NY) at no additional cost. The plan covers expenses associated with identity theft (e.g. attorney's fees, lost wages, affidavits, long distance charges, copies, etc.), and all at no charge to the Member. Members also receive comprehensive identity restoration services, whereby a professional investigator will help recover a Member's identity, clear their name, and restore their credit rating back to pre-theft status at no additional charge. Credit services are provided by TransUnion Interactive, Inc.
