While the Basic Plan may meet the needs of most sponsoring organizations, working with Your Wellness Partners also affords you the flexibility to choose from a wide variety of available services to construct and deliver the unique services your clients demand. As a result, the Basic Plan should be viewed as just one possible combination of services.

To illustrate the value delivered by the Basic Plan, the average cost of a dental visit in 2004 was $560¹. Savings from using just the dental discount in the Basic Plan alone would exceed the entire cost of the plan!

HealthWebTools.com is a Wellness web site that empowers Members to manage their health more effectively. It features an online Health Risk Assessment, 24/7 Live Nurse Chat, symptom checker, disease and condition center, resources such as weight and health information tools, quizzes, glossaries, self-care tools, lifestyle topics, a personal health record, prescription drug guide, and more! Utilizing the nationally-recognized University of Michigan online Health Risk Assessment, Members can identify areas in which they are effectively managing their health as well as ways to improve it. 24/7 Live Nurse Chat allows Members to ask questions about conditions, procedures, medications, or any other health issue they may be interested in, and learn about community resources that provide additional information and support. Online health coaching programs provide helpful tips and empower Members to create personalized action plans to reduce health risks while improving nutrition, implementing a fitness program, managing stress, and modifying other lifestyle behaviors that impact health and well being.

Dental Plan

The Dental Plan provides a nationwide network of over 24,000 credentialed providers, including dentists and dental care specialists. Members can save on routine and preventive care in addition to more extensive treatments such as fillings, crowns, root canals, orthodontics, and cosmetic dentistry. Although discounts may vary based on geographic region, the average range of discount is between 10% - 50%.

Vision Plan

The Vision Plan provides Members 20% to 60% off on eyeglasses, contact lenses (excluding disposable) and other retail eyewear items nationwide. The network of over 12,000 providers is appropriately licensed, certified or qualified under state law. Providers include several national and regional retail chains, such as Pearle Vision, Lens Crafters, Sterling Optical, Sears Optical, JC Penny Optical, and TLC Laser Vision Centers. In addition, Members save 10% - 30% on eye examinations and surgical procedures at participating providers and 10-40% on replacement contact lenses ordered through the mail. The Vision Plan guarantees satisfaction and the lowest price. If a Member finds a lower price anywhere else on the exact same pair of prescription eyeglasses purchased at a participating location within 30 days, the difference will be refunded. Satisfaction in the Vision Plan and the mail order service is fully guaranteed. If for any reason a Member is not happy with a purchase, the merchandise can be returned within 30 days for an exchange or full refund.

Hearing Plan

The Hearing Plan provides a nationwide network of licensed audiologists. Members receive a 10% - 20% discount from a network provider's ordinary fees for hearing aids. Free annual cleaning and check of hearing aids purchased through the program is also included.

Prescription Drug Plan

The Prescription Drug Plan is offered through a nationwide network of pharmacies, including the following major chains: Albertsons, Brooks, Costco, K-Mart, Kroger, Pathmark, Publix, Rite-Aid, Sav-On, Stop & Shop, Target, and Winn Dixie. The benefit provides a discount of up to 30% on most brand name and generic prescription drugs. In addition, a mail order prescription drug service is available, offering discounts between 15% - 50% on most prescription drugs.

Chiropractic Plan

The Chiropractic Plan provides one of the largest nationwide networks of licensed chiropractors. The benefit includes a free initial consultation, and savings of 20% to 40% on the chiropractor's ordinary fees.

Vitamins and Nutritional Supplements Plan

The Vitamins and Nutritional Supplements Plan is offered nationwide through a mail order service at savings of 25% - 50% of suggested catalog prices on vitamins, non-prescription medications, beauty supplies, and nutritional supplements.

Diabetes Management Plan

The Diabetes Management Plan provides savings on home delivery of diabetes testing supplies (with no charge for shipping), a friendly reminder when it's time to reorder, and customer service representatives who are both knowledgeable and courteous. Each kit uses custom made trays from the Member's personal impressions.

LASIK Network

The LASIK Network allows Members to take advantage of the LASIK eye surgery network and save 40% to 55% off the overall national average charge for LASIK at 400 participating provider locations nationwide. Each provider is a Board Certified Ophthalmologist who uses FDA-approved LASIK technologies.
